Resident Care Coordinator (RCC)Valencia at Draper Assisted Living & Memory CareDepartment: Personal Care Reports To: Wellness Director (RN) & Executive Director FLSA Status: Non-Exempt Status: Full-Time Schedule: Typically Monday-Friday. Weekend, holiday, and on-call availability required as needed.Position SummaryValencia at Draper is seeking an experienced, compassionate, and highly organized Resident Care Coordinator (RCC) to join our leadership team. The RCC plays a vital role in ensuring residents receive exceptional, person-centered care while leading, developing, and supporting our caregiving staff.Working closely with the Executive Director and Wellness Director, the RCC oversees daily care operations, staffing, scheduling, resident assessments, quality assurance, regulatory compliance, and caregiver development. This position requires a servant leader who is passionate about serving seniors, building strong teams, and creating a culture of accountability, ownership, and excellence.Minimum Requirement: Six months of leadership experience in Assisted Living or Memory Care (Resident Care Coordinator, Lead CNA, Lead Med Tech, or similar supervisory role).Essential ResponsibilitiesResident CareEnsure residents receive safe, compassionate, and individualized care.Assist with resident assessments and ensure care plans accurately reflect residents' current needs.Monitor changes in resident condition and coordinate appropriate interventions.Collaborate with physicians, pharmacies, families, hospice, home health, therapy providers, and outside healthcare professionals.Coordinate resident appointments, laboratory work, treatments, and follow-up care.Monitor medication administration processes and ensure timely communication regarding medication changes.Participate in resident move-ins, assessments, and transitions of care.Assist with determining and updating resident levels of care.Staff LeadershipRecruit, interview, hire, onboard, and retain outstanding caregivers.Create and manage caregiver schedules to meet resident needs while maintaining labor budgets.Supervise, coach, mentor, and develop caregivers and medication technicians.Conduct regular performance evaluations and provide ongoing coaching and feedback.Lead corrective action and accountability conversations when necessary.Ensure all caregivers receive proper orientation, training, competency validation, and continuing education.Foster a positive culture built on teamwork, professionalism, accountability, and resident-centered care.Participate in the on-call rotation by responding to staffing emergencies, arranging coverage, or working shifts when necessary.Quality Assurance & ComplianceMonitor quality of care through observations, audits, documentation reviews, incident investigations, and resident outcomes.Ensure resident records, care plans, and documentation remain accurate, complete, and compliant.Participate in Quality Assurance meetings and implement continuous improvement initiatives.Ensure compliance with state regulations, company policies, and community standards.Maintain employee records in accordance with Department of Health and Labor requirements.Assist with surveys, inspections, and regulatory readiness.OperationsCoordinate staffing based on resident acuity and levels of care.Collaborate with the Executive Director and Wellness Director to ensure service plans accurately reflect resident needs and billing.Monitor staffing efficiency while maintaining high standards of resident care.Assist with orientation, in-services, and ongoing staff education.Support resident move-ins and transitions.Assist with community tours and marketing efforts as needed.Perform additional duties as assigned.Supervisory ResponsibilitiesThe Resident Care Coordinator directly supervises caregiving staff and is responsible for:Interviewing and hiring employees.Training and onboarding new staff.Assigning and directing daily work.Monitoring performance and providing coaching.Conducting performance evaluations.Recognizing strong performance.Addressing performance concerns through coaching and corrective action.Building a positive, accountable, and engaged team.QualificationsRequiredMinimum six months of leadership experience in Assisted Living or Memory Care.Experience supervising caregivers or CNAs.Strong understanding of resident assessments, care plans, and quality assurance.Excellent leadership, communication, and organizational skills.Ability to prioritize multiple responsibilities in a fast-paced environment.Proficiency with electronic documentation systems and Microsoft Office.Ability to participate in an on-call rotation and work weekends when necessary.PreferredCNA certification or healthcare-related certification.Experience working in Memory Care.Core CompetenciesSuccessful candidates demonstrate:Compassion and resident-centered careLeadership and accountabilityIntegrity and professionalismExcellent communication skillsCritical thinking and sound judgmentStrong organizational skillsTeamwork and collaborationAdaptability and initiativeAttention to detailCommitment to continuous improvementPhysical RequirementsAbility to walk throughout the community for extended periods.Ability to assist during resident emergencies.Ability to lift up to 50 pounds with assistance when appropriate.Ability to bend, reach, stoop, and assist residents as needed.Ability to respond quickly to resident and staffing needs.Why Join Valencia at Draper?At Valencia at Draper, we believe exceptional resident care begins with exceptional leadership.
