What You’ll Do Conduct patient admission assessments and develop care plans. Perform supervisory visits to monitor patient care and progress. Educate patients and families on medications, conditions, and care plans. Document visits accurately and communicate changes in patient condition. Collaborate with physicians and the healthcare team to ensure quality care.
