Work Environment

This position is set within a home care environment, allowing Physical Therapists to deliver personalized treatment in settings that are comfortable for patients. Key Responsibilities

• Conduct thorough evaluations of each patient’s physical condition and functional abilities to create individualized treatment plans.
• Implement orthopedic treatment strategies, utilizing advanced therapeutic techniques and modalities, including manual therapy and therapeutic exercise.
• Assess and monitor patient progress through regular reevaluations, adjusting care plans as necessary based on observed improvements or challenges.
• Collaborate with patients to establish and track their therapy goals, ensuring that the methods used effectively lead to improved mobility and pain reduction.
• Educate patients and their families on injury prevention strategies and the importance of ongoing physical health and wellness.
• Maintain accurate and timely documentation of patient encounters, treatment plans, and progress notes in accordance with legal and organizational guidelines.
• Consult and work alongside interdisciplinary team members, including physicians and other healthcare providers, to foster a holistic approach to patient care.
• Actively engage in staff training, team meetings, and professional development programs to enhance your skills and knowledge in orthopedic therapy.

Patient Population / Client Focus

In this role, you will primarily focus on orthopedic patients recovering from a range of musculoskeletal conditions, including but not limited to orthopedic surgeries, sports injuries, and chronic pain syndromes. You will play a crucial role in helping these patients regain functionality, improve their quality of life, and achieve their personal rehabilitation goals.

Qualifications

• Valid Bachelor's, Master’s, or Doctoral Degree in Physical Therapy from a CAPTE-accredited program.
• A current and active Physical Therapy license from the State of New Jersey.
• A genuine interest and experience in treating orthopedic populations, particularly in a home care setting.
• A minimum of two years of experience in a Physical Therapist role, preferably with a focus on orthopedic care.

Preferred Experience

• Previous work in diverse therapeutic environments, including outpatient clinics or rehabilitation centers.
• Familiarity with a variety of treatment modalities and technologies that can enhance patient recovery.
• Experience in a collaborative practice model that includes interdisciplinary teamwork in rehabilitation.
