Required Skills:

• Valid State of Connecticut Registered Nurse license
• Associate's degree in Nursing (Bachelor's preferred)
• CPR certification at the healthcare provider level or eligibility to obtain within 90 days
• Strong clinical skills including medication administration, triage, and patient assessment
• Proficiency in electronic health records and healthcare technology
• Exceptional communication abilities, including proficiency in Spanish or additional languages (preferred)
• Ability to work independently with minimal supervision and thrive in a fast-paced environment
• Knowledge of population health and patient education principles

Nice to Have Skills:

• Bachelor's degree in Nursing
• Experience working with socioeconomically diverse, multiethnic populations
• Bilingual proficiency in Spanish or other languages
• Experience with chronic care management and care coordination
