This position is not eligible for remote work, as the successful candidate will actively engage with patients and other healthcare team members in person.

Duties and Responsibilities

• Assess patient’s health problems and needs, develop and implement nursing care plans, and maintain medical records.
• Administer nursing care to ill, injured, convalescent, or disabled patients.
• Advise patients on health maintenance and disease prevention or provide case management.
• Liaise with physicians and other team members to plan and evaluate patient care plans.
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Guide and supervise health care personnel like LPNs and home health aides.
• Manage a caseload of patients, ensuring all patients receive the required care as per the established guidelines.
• Maintain accurate, detailed reports and records of patient conditions and treatments.
• Monitor, record, and report symptoms and changes in patients' conditions in a timely manner.
• Order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions.
• Coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
• Direct and coordinate activities of nurses, assistants, specialists, therapists, and other medical staff.
• Ensure compliance with laws and regulations in health care.
• Participate in educational opportunities to update knowledge and skills.

Requirements

• Must hold a valid RN license in Oregon.
• Preferred experience in home health care settings.
• Expertise in managing a caseload of Hospice patients.
• Ability to coordinate with LPNs and other members of the hospice team.
• Proven ability to provide compassionate and effective care to patients.
• Strong organizational and communication skills.
• Must be able to work efficiently without direct supervision.
• Demonstrated capability in making clinical judgments in a high-paced, dynamic environment.
• Knowledge of computer operations and proficiency in the use of basic word processing, data entry and clinical information systems.
• Adept at handling and resolving challenging clinical situations.
• Commitment to continuing education and professional development.
• Flexible with work hours, able to handle weekend or evening assignments if necessary, and available to respond to emergencies.
• High ethical standards and professionalism, with a commitment to patient privacy.
