Licensed Practical Nurse LPN

Responsibilities:

Provide direct patient care including wound care, medication administration, and vital sign monitoring in home settings

Assess and document patient health status, progress, and changes in condition accurately and in a timely manner

Administer prescribed medications and treatments in accordance with physician orders and care plans

Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of patient care

Educate patients and family members on care plans, medications, and health management techniques

Maintain accurate and up-to-date patient records in compliance with state and federal regulations

Monitor patients for adverse reactions and report significant changes to supervising RN or physician promptly

Requirements:

Active Licensed Practical Nurse (LPN) license in the state of Maryland

Prior home healthcare, long-term care, or clinical nursing experience preferred

Strong knowledge of nursing procedures, medication administration, and patient assessment

Current CPR/BLS certification required

Excellent communication, documentation, and interpersonal skills

Ability to work independently and manage time effectively in a home care setting

Reliable transportation and valid driver's license to travel to patient homes in the Baltimore area
