Growing together.We are currently hiring a Homecare Nurse for our Optum Women's and Children's Health - Homecare. This position will provide skilled nursing care in a home care environment. Assess, plan, and document the homecare nursing process; educate and provide instruction to patients and their family.Travel: This position will travel up to 2-2.5 hours to patients homeSchedule: Once you get the referral the patient has to be seen within 24-48 hours where you can work with the patient to self schedulePrimary Responsibilities:Positively represent the Company to both internal and external customers; always maintain a professional and courteous mannerUphold the Optum core values in the conduct of workAdhere to Company privacy guidelines; ensure compliance with local, state, and federal regulationsProvide services according to Optum policy and the prescriber's plan of treatmentIndividualize patient assessments based on care/services being provided and patient needEnsure interventions respect and encourage the patient's ability to make choicesRespect each patient's rights and conduct business in an ethical mannerEducate patients and families with a systematic approach, promoting patient understanding of treatment and services, health status, coping ability and patient/family involvement in the continuum of care or serviceInitiate the plan of treatment/care pathway based upon the analysis of information obtained and within scope of practice and certificationModify the plan of care, as appropriate, based on re-assessment, change in prescriber orders, the patient's need for further care or services, and the achievement of identified goals within scope of practice and certificationCoordinate care with the patient services centers, pharmacists, and other healthcare professionals to promote the continuum of care or serviceAccurately communicate (written/verbal) patient status information to prescribing physician in timely manner and provide necessary clinical information to Optum pharmacists and other healthcare professionals as necessaryComplete clinical record documentation per Optum policy in a timely, complete, and accurate mannerProvide service-related clinical support to key physicians/hospitals through personal contacts and in-services, as directedParticipate in holiday and weekend duties as scheduledMust maintain a current CPR certification and obtain an annual screen and TB test and influenza immunization in accordance with Optum policyMust comply with random drug testingEnsure senior management decisions and directives are carried out Routine travel requiredMay be asked to participate in training or act as preceptorAdditional duties as assignedSupervisory Responsibilities:Home health administrator responsibilities as assignedSpecial project management as assignedAdditional responsibilities as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current unrestricted nursing license (RN) for state of residence1+ years of obstetrical nursing experienceProven ability to maintain competency on all services provided according to policyProven technology knowledge to include desktop/laptop computer proficiency; use of analytical software, Microsoft Office, Outlook (internet and email). Attend in-services on applicable technology enhancementsAdditional state required qualifications may applyDriver's License and access to reliable transportationPreferred Qualifications:BSNProven exceptional communication skills (written and verbal)Proven ability to deal with patient/family conflict resolution and grievancesProven ability to work with individuals with disabilitiesPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29 to $52 per hour based on full-time employment.
