About the Position


The primary goal of the Home Health Registered Nurse is to help patients return to their highest practicable level of independence and well-being within the community through effective self-management of their health conditions.


When independence is not achievable, the nurse focuses on preparing the patient, family, and caregivers for a seamless continuum of care within the home or other community-based healthcare systems.

Qualifications

• Education: Graduate of an accredited, approved school of professional nursing.
• Licensure: Active Registered Nurse (RN) license in the State of Colorado in good standing.
• Experience: Minimum of one (1) year of experience as a Registered Nurse. Home health experience is highly preferred.
• Skills: Familiarity with OASIS documentation is preferred. Must be able to communicate effectively in the patient's language and document all patient-related activities accurately.
• Transportation: Valid driver's license and reliable transportation.
