Nurse Practitioner/Physician Assistant Primary Care SNF/ALF Location: Somerset, KY, 42501 Job Summary The Nurse Practitioner/Physician Assistant Primary Care SNF/ALF will provide primary care services to patients in skilled nursing facilities and assisted living facilities. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to provide high-quality care to patients. Responsibilities Conduct comprehensive assessments of patients' health status Develop and implement treatment plans Order and interpret diagnostic tests Prescribe medications and other treatments Provide patient education and counseling Collaborate with physicians and other healthcare professionals to provide coordinated care Maintain accurate and complete medical records Participate in quality improvement initiatives Adhere to all applicable laws, regulations, and standards of practice Qualifications Master's degree in nursing or physician assistant studies Current state licensure as a nurse practitioner or physician assistant Certification as a nurse practitioner or physician assistant Experience in primary care, preferably in a skilled nursing facility or assisted living facility Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong problem-solving and critical thinking skills Ability to maintain confidentiality and adhere to ethical standards #J-18808-Ljbffr
