Job Overview Health is seeking a Nurse Practitioner to work in collaboration with our Medical Oncologist at Northern Dutchess Hospital in Rhinebeck, NY. The model of care is multidisciplinary, integrated, and disease‑specific. In this role you will coordinate, oversee, and provide advanced nursing care to medical oncology patients and serve as a Clinical Patient Navigator for this population. Job Highlights Works collaboratively with physicians and nurses to provide comprehensive care to patients. Conducts comprehensive and episodic health history and physical assessment of patients. Oversees treatment infusions and supportive care needs. Writes orders within the limits of the practice agreement. Maintains clinical documentation according to group practice standards. Participates in clinical quality and research as needed. Educates patients, families, and caregivers on treatment plans. Participates in learning opportunities.
