Clinical Staff Pharmacist - Home Infusion (Hybrid, Middleton)

UW Health Care Direct infusion pharmacy is seeking a licensed pharmacist to perform clinical and distributive pharmacy duties for home infusion therapies in Middleton, WI. You will collaborate with prescribers and Care Direct staff to provide safe, effective drug therapy and maintain patient records.
The role is 100% FTE with 10-hour days, four days per week, on-call requirements, and a hybrid arrangement after training, 50% on-site in Middleton.
