Advanced Practice Registered Nurse

  1. Job duties/scope of work:
Duties
  • dmits, assesses, diagnoses and formulates initial treatment plans for newly admitted patients, with attention to psychiatric, medical, substance abuse, and legal problems.
  • Conducts semi-structured interviews performing comprehensive highly skilled clinical psychiatric assessments that require extensive knowledge of various medical and psychiatric conditions, complications, treatments and therapies for the purpose of eligibility determination, diagnosis, treatment planning, and treatment delivery.
  • Collects collateral information (e.g., criminal record, pre-sentence reports, previous mental health evaluations, interviews with family members, progress notes, etc.) in forming an assessment.
  • Employs the current Diagnostic and Statistical Manual of Mental Disorders and the International Statistical Classification of Diseases (ICD) to formulate diagnosis, APA accepted references and the AMHD assessment tools based upon best Standard of Practice/Evidence-Based Practice methods.
  • Provides psychopharmacological intervention, as appropriate and clinically indicated, and documents the rationale for choice of medication, risks and benefits, side effects, target symptoms and efficacy.
  • Provides medication management, including administration of subcutaneous, intradermal and intramuscular medication to consumers as needed.
  • ssumes full clinical responsibility for psychiatric treatment and prescribes psychiatric medications for assigned cases. Consults with the psychiatry chief or designated psychiatrist as needed.
  • Provides clinical guidance for the multidisciplinary team and other staff members by developing, integrating, and approving all case formulations and consumer treatment plans as well as authorizing all treatment interventions included in the treatment plan for assigned patients.
  • Performs treatment services within the realm of psychiatric and family clinical nursing expertise, focusing on psychiatric and medical co-morbidity management including treatment planning, medication, and psychotherapy including integrated services for those with co-occurring substance use disorders and forensic encumbrances.
  • Maintains contact with out-patient facilities to ensure continuity of care, including consulting with the outpatient treating psychiatrist or APRN II at admission and discharge from HSH.
  • Identifies service needs and assists in resource identification and formulation of services and resources based on the identification of problems/concerns within specific medical/nursing diagnosis, which improve consumer outcomes.
  • Performs field visits as necessary by following up on patients who are pending discharge or have been discharged from the hospital to assist with the patient's transition to the community.
  • Initiates and completes the information needed to request an Order to Treat (OTT) as the patient's attending psychiatrist or APRN II and shall be present at the hearing to present information relative to the patient's need for medical treatment and the proposed treatment plan, as needed.
  • Provides psychiatric and pharmacological services coverage in the absence of a psychiatrist if needed.
  • Provides timely and accurate documentation in the clinical record and in the AMHD management information system of activities and other related data and information as required by the AMHD and in accordance with the policies, procedures, standards, directives, initiatives and planned outcomes of the AMHD and the HSH.
  • Prepares admission, transfer and discharge summaries and other reports and letters as required by HSH policies and procedures in a timely manner.
Other duties
  • Prioritize and organize assigned tasks for the APRN's subordinates to ensure adequate and appropriate distribution of work tasks for HSH personnel.
  • Delegate assignments to nursing staff.
  • Provide consultations to other providers and staff.
  • Document clinical findings and recommendations in accordance with departmental policies and procedures.
  • Work collaboratively with inter-disciplinary team members and other clinical providers such as the person served may have (e.g., community primary care and/or psychiatric providers).
  • ttend and provide in-service training as needed.
  • Participates as a member in function team and committee meetings in Section's and/or Unit's Quality Management review processes, performance improvement activities and decisions, and continuously strives to improve the quality of consumer care.
  • Provides written reports, statistical forms or data as requested.
  • Maintain a clean, well-stocked, safe environment for patients and staff.
  • ttends in-service training, staff meetings, relevant educational conferences and workshops, and case conferences.
  • Responds to emergency call-outs to provide specific assistance in his/her professional capacity as needed.
  1. Equipment to be used
Phone, fax machine, computer and specialized software, such as, Electronic Medical Record, word processing and video conferencing.
  1. Minimum Requirements:
    1. Knowledge
  • Thorough knowledge of principles, methods and techniques of psychiatric nursing and the nursing concepts and theories that are guides for their use; emotional factors influencing the behavior and attitudes of individuals.
  • Thorough knowledge of psychopharmacology and familiarity with the most current psychiatric medications, their applications, indications, contra-indications, etc.
  • sound knowledge of the dynamics of human behavior, including considerable knowledge of the nature of normal and abnormal behavior, the causes and consequences of deprivation, the undermining, stabilizing and strengthening factors in social circumstances and interaction, and the cultural and psychological implications of family disunity, unemployment, illness, etc.
  • Knowledge of best Standard of Practice/Evidence-Based Practices relative to community-based advanced psychiatric nursing, medication management and clinical treatment services.
  • Knowledge of the principles, methods and techniques of the integrated treatment and recovery of consumers with co-occurring substance use disorders, forensically encumbered consumers, and the delivery of culturally informed, sensitive and responsive services to diverse cultural populations.
  • Knowledge of chemical dependency, dual diagnosis, psychopathy, and cognitive impairments.
  1. Minimum skills/abilities
pply practical nursing principles and practices; read, write, speak, understand and communicate effectively with others in English; maintain records and prepare reports; respond quickly and take appropriate action in emergencies; follow oral and written instructions; establish and maintain effective relationships with other staff, patients and others.
  1. Minimum education, certificates and licensing
  • Hold a current license in accordance with Hawaii Revised Statutes and Administrative Rules to practice in the State of Hawaii as an advanced practice registered nurse and licensed for prescriptive authority.
  • Hold current certification by the American Nurses Credentialing Center as a Psychiatric Mental Health Nurse Practitioner-BC or Psychiatric Mental Health Clinical Nurse Specialist -BC and maintains this certification.
  • Possession of valid State of Hawaii Narcotics Enforcement Administration Registration and Federal Drug Enforcement Administration Registration.
  • Possession of a Valid Type 3 Driver's License is desirable.
  1. Minimum experience/training
Must meet the experience requirements for the Advanced Practice Registered Nurse II class specifications and have two (2) years of progressively responsible professional clinical supervisory experience as indicated by the complexity, scope, and variety of operations supervised, the type and intensity of services offered by the supervised units, the range and number of subordinates supervised, and kind and degree of supervision exercised over subordinate positions is preferred.
  1. Physical Requirement:
Must be physically able to perform the essential duties of the position. The general types of physical abilities include: sensory; coordination and dexterity; strength and stamina; and mobility.
  1. Minimum computer software skills/knowledge:
Microsoft Word and Excel.
  1. Hours of work: 40 hours per week
Direct Care: 3 Shifts
  • 6:45 a.m. to 3:15 p.m.
  • 2:45 p.m. to 11:15 p.m.
  • 11:00 p.m. to 7:00 a.m.
Expected length of assignment: 1 year
