We also have an immediate opening for a Bilingual Case Manager (English/Burmese).

What You'll Do

Client Assessments & Care Plans - Evaluate needs and design personalized care plans
Coordinate Services - Partner with nurses, caregivers, and providers for seamless support
Advocate for Clients - Be a trusted voice for patients and families
Maintain Compliance - Keep records accurate and services aligned with state and federal guidelines
Crisis Support - Step in quickly when urgent issues arise
Monitor & Adjust - Track progress and adapt care plans to improve outcomes

What We're Looking For

Excellent communication & relationship-building skills
Strong organization and time-management abilities
Knowledge of Medicaid, Medicare, and Indiana home care guidelines
Background in case management, social work, or healthcare
Tech-savvy with EHR or case management systems
Bilingual (English/Burmese) is a plus-immediate need!
