What You Will Do

• Transport participants to community, recreational, and other activities that promote skill development and personal growth.

• Provide appropriate supervision and support to ensure participant health and safety while in the community.

• Lead and facilitate group sessions focused on practical daily living skills, including communication, teamwork, problem-solving, and community participation.

• Actively engage participants in activities by encouraging participation and maintaining a positive, supportive, and meaningful environment.

• Assist individuals in developing greater independence through work, recreational, social, and daily living skills.

• Support participants in identifying personal goals and developing strategies to work toward those goals.

• Encourage individuals to make informed choices and participate in activities that reflect their interests and preferences.

• Serve as a positive role model while building trusting and supportive relationships with participants.

• Provide person-centered support based on each participant's individual needs, interests, abilities, and goals.

• Support participants in achieving their personal goals and improving their overall quality of life.

• Document participant progress and activities in accordance with program requirements and organizational policies.

• Communicate relevant participant needs, progress, and concerns to the appropriate members of the support team.

Minimum Qualifications

Required:

• Ability to provide compassionate, person-centered support to individuals with developmental disabilities.

• Strong communication, interpersonal, and organizational skills.

• Ability to work effectively as part of a team and independently when appropriate.

• Ability to safely transport and support participants in community settings.

• Must have completed the orientation period of 3–6 months in the current position, where applicable.

• Must be in good standing in the current position, where applicable.

• Must be current with all required mandatory trainings.

• Smartphone required.

Preferred:

• Experience working with individuals with intellectual and developmental disabilities.

• Experience in direct support, human services, education, recreation, healthcare, or a related field.

• High school diploma/GED or relevant education and experience.

Work Location

Starts and ends in Westbury, NY (Nassau County)

The position includes community-based activities and transportation to various locations as part of the participant support program.

Work Hours / Schedule

Full-time, Monday–Friday

8:00 AM–4:00 PM

Consistent weekday schedule with no regular overnight shift indicated.

Pay

$20.50 per hour

Paid Time Off
