Seeking a Rehabilitation Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center. Full-Time Employment: 40 hours per week Monday - Friday: 8:00am - 5:00pm Location: 4451 E. SMI DIFFERENTIAL Duties Include: Assistclients with identifying treatment and recovery goals, and providesupportive services to help clients reach their employment, education, and community involvement goals. Implementservices identified inIndividual Service Plans, and coordinate rehabilitation-related services for clients served on the clinical team. Communicateand documentclient progress toward their recovery. Embracethe recovery philosophy and promotes a safe recovery environment for clients. Usebrief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Engage and providedirect supportto clients’ natural support systems. Workin partnership with co-located Employment Specialists and RSA Vocational Rehabilitation Counselors, community program providers, and other parties involved to assist clients inmaintaining employment, education, and other meaningful daily activities. Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessmentwith minimum 25wpm. Knowledge in use of Electronic Health Records - NextGen is a plus. Bilingual in Spanish/ASL is desired. Excellent oral and written communication skills. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role. Must pass a TB Test.
