RN - Case Management

Case Management RN - Acute Care

This is a 13-week contract position.

Day Shift: 5x8 (07:30 - 16:00)

The Case Management RN will be responsible for care coordination and discharge planning within a Level II Trauma Center. This role requires strong acute care experience and a diverse background in case management across Med Surg and Emergency Departments.

Responsibilities:

• Provide skilled case management services focusing on care coordination and discharge planning for patients in an acute care setting.
• Collaborate with interdisciplinary teams to optimize patient care and ensure appropriate discharge planning.
• Work across multiple areas of the hospital, including Colby and Pacific Campuses.
• Monitor patient progress and adjust care plans as needed in accordance with CMS guidelines.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of acute care RN experience.
• Strong background in discharge planning and care coordination.
• Current BLS certification required.
• WA RN License required (compact license accepted).
• Experience with EPIC charting system preferred.
