You will collaborate with care team members to identify, address and treat mental health needs of patients within the Allina Health system. You will have a team at your home clinic as well as a team of Mental Health Consultants located at other Allina Health clinics. Provides crisis consultation and de-escalation to patients in person and/or virtually Collaborates with Primary Care/Women’s Health teams and community Supports patients and primary care team/mental health providers in achieving patient care goals. Reinforces providercommunication aboutplan of care. Coordinates care for patients across the primary care, specialties, and mental health service lines Assess conditions related to crisis and life events. Triage patients with known mental health issues and provide same-day and short-term ongoing access to mental health therapy Consults,refersand coordinates interventions in cases of suspected or actual situations of child or adult abuse and neglect or forms of domestic violence or sexual abuse. Identifiesneeds for community or public agency services Required Qualifications Master’s degree in Social Work or related field Preferred Qualifications 2 to 5 years in licensed practice Licenses/Certifications Licensed Ind Clinical Social Worker - MN Board of Social Work required upon hire or Must meet at least ONE of the requirements below: Current BLS certification from the American Heart Association Current BLS certification from the American Red Cross Allina in-house BLS training (within 30 days of hire) Physical Demands Sedentary: Lifting weight up to 10 lbs. occasionally, negligible weight frequently Pay Range Pay Range: $35.61 to $48.81 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
