Key Responsibilities
- Conduct comprehensive nursing assessments and develop individualized hospice care plans in collaboration with the interdisciplinary team (IDG)
- Provide ongoing clinical monitoring and updates to reflect changes in patient condition
- Coordinate patient care activities including scheduling, medication management, and physician communication
- Deliver education and emotional support to patients and caregivers
- Respond to patient needs during scheduled visits and on-call assignments as required
- Participate in interdisciplinary team meetings and care planning discussions
- Supervise LPNs, LVNs, and hospice aides as needed
- Ensure accurate, timely clinical documentation in compliance with regulatory and organizational standards
- Support continuity of care through coordination across providers and services
- Active Registered Nurse (RN) license in the state of Massachusetts
- Associate Degree in Nursing required (BSN preferred)
- Minimum 1 year of nursing experience preferred (hospice, home health, med-surg, or geriatrics preferred)
- Current CPR certification preferred
- Strong clinical assessment and care planning skills
- Ability to work independently in a field-based setting
