Nurse Practitioner Or Physician Assistant

Tanner Health is seeking a full-time Nurse Practitioner or Physician Assistant to join the Tanner Medical Group Bridge Clinic team which is a transitional care facility designed to "bridge" the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP).

Job Duties & Responsibilities:

• Provide primary care services including medication management, chronic disease monitoring, coordination of care with specialists and patient education.

Job Eligibility Requirements:

Education & Licensure:

• Requires completion of an accredited Nurse Practitioner program, with a master's degree in nursing OR master's degree from an accredited Physician Assistant program
• Active GA APRN license OR GA Physician Assistant license or meet criteria for obtaining licensure.
• Must be board certified as a Physician Assistant.

Experience:

• Prior experience in adult medicine is preferred.
• New graduates are welcome to apply.

Schedule & Location:

• Full-time, Monday – Friday, 8am – 5pm.
• Position will be located require shifts in Carrollton and Villa Rica Bridge Clinic locations.
