• 10-20 Patients Seen at a Time

• Only 2 Call Days per Month

About the Role

• Community-based physician rounding in subacute skilled nursing facilities

• Flexible scheduling - you control your day

• Low census: 10-20 participants seen once weekly

• Lead clinical oversight and quality for provider services

• Collaborate with an experienced, supportive interdisciplinary team

• Serve as a physician leader and mentor to NPs and other providers

• Champion person-centered care for frail elderly participants

Compensation & Benefits

• Competitive physician salary commensurate with experience

• $5,000 annual CME allowance

Qualifications

• MD or DO with active state medical license

• Board Certified in Internal Medicine or Family Medicine (geriatrics preferred)

• DEA registration and ability to maintain hospital privileges

• Experience in primary care with frail or elderly populations

• Passion for mission-driven, whole-person care
