Traveler Requirements

Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quick! This form needs to be 100% accurate and have all request noted in the highlights section.

• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page, must be less than 7 days.
• RTO will not be approved in the first week of assignment or holidays.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.
• Please let us know if the traveler withdraws or is no longer available.

Requirement Description:

• Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission
• 24 months minimum of current Cardiac Cath Lab experience - REQUIRED
• Must have RN Cath lab and IR Angio experience - REQUIRED
• Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
• Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**
