Key Responsibilities As a Licensed Clinical Social Worker, you will be integral in providing acute psychosocial assessments and developing tailored care plans for a diverse client base. Responsibilities include: Engaging directly with clients experiencing major mental illnesses or severe emotional disorders, both through virtual appointments and community outreach. Collaborating with a multidisciplinary team to advance client-centric goals and deliver effective mental health interventions. Building therapeutic rapport with clients, employing trauma-informed approaches that respect individual beliefs and backgrounds. Monitoring and maintaining ethical standards in practice, aligning with the professional code of conduct as defined by the ethical guidelines of the clinic. Conducting brief, evidence-based screenings and arranging for specialized evaluations as deemed necessary. Implementing integrated care plans that ensure connectivity to essential services, facilitating effective communication among family members and healthcare providers. Providing crisis intervention as needed, assisting clients through critical moments with a focus on safety and recovery. Managing a varied caseload which might include individual and group therapy, ensuring responsive treatment aligned with client needs and goals. Accurately documenting patient progress, facilitating seamless reporting, and ensuring timely submissions of necessary data. Participating in quality improvement initiatives and committees to continuously enhance service delivery and patient care standards. Patient Population / Client Focus This role primarily involves working with adults and families who face significant psychosocial challenges, including those experiencing distress due to chronic illness or life-limiting conditions. Your expertise in addressing mental health issues will play a critical role in improving their quality of life and providing them with the necessary support during difficult times. Qualifications To be successful in this role, candidates must meet the following criteria: A Master's degree in Social Work (MSW) or a related field is mandatory. Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon. A robust understanding of assessment, diagnosis, and evidence-based treatment methodologies for mental health conditions. Exemplary verbal and written communication skills, with an ability to relate effectively to a diverse population. Strong organizational skills coupled with a proactive approach to planning and service delivery. Preferred Experience While not mandatory, the following qualifications will be viewed favorably: A minimum of 2 years experience in a clinical social work setting, ideally within hospice or palliative care contexts.
