Registered Nurse Home Care Coordinator (RN)
Our client is a growing healthcare organization that specializes in working with the geriatric population. We are looking for a full-time Registered Nurse Home Care Coordinator (RN) to provide care to patients in their homes in the Lowell, MA area. The client offers the following compensation package:
- $93k- $100k Salary Based on Experience
- Health Insurance- Employer Pays 80% of Premium

Responsibilities:
- Monday- Friday 8:00am- 4:00pm No Nights No Weekends
- Responsible For Helping Patients Establish Services
- See Patient in Their Home, Complete Admin From The Clinic
- If No Home Care Coordination Patients Are to be Seen, RN Will Help in the Clinic
- Periodically Check in to Make Sure Appropriate Services Are Still in Place
- Extremely Limited On Call- Every 12-14 Weeks
Requirements:
- MA RN Licensed
- New Grads Welcome
- Covid Vaccination Required
- CPR Certified
- Works With Team of Nurses
- Build Strong Relationships With Participants
- Must Have Great Communication Skills
