Reliable means of transportation with proof of auto insurance and valid Michigan Driver's License Duties and Responsibilities: Monitor individual treatment and support plans, notify the appropriate I/DDCare Coordinator when needed. Gather and compile personal and demographic information. Understand rules and regulations governing care and support for individuals with developmental disabilities. Attend planning meetings using person-centered principles and coordinate supports and community services. Coordinate transitions into personal residences and broker services/supports. Linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of supports and other community services/supports Coordinate with Medicaid and other health care providers.
