Nurse Practitioner (Primary Care) Position Summary The Nurse Practitioner will work with a collaborative clinical care team to provide evidence-based primary and family care. Responsibilities include patient assessment, diagnosis, treatment, chronic disease management, preventive care, health counseling, and care planning to improve patient outcomes. Key Responsibilities Provide comprehensive primary and family care, including assessment, diagnosis, treatment, and follow-up. Perform routine exams, preventive screenings, and diagnostic evaluations. Educate patients on healthy lifestyle choices, wellness, and chronic disease management. Coordinate care with physicians and other healthcare professionals. Manage patient appointments, documentation, and administrative duties such as insurance verification, inventory, and maintaining the clinic environment. Qualifications Master's Degree in Family Nurse Practitioner (FNP) program. Current National Board Certification and active State NP/APRN license. Minimum 1 year of recent Primary Care experience in family medicine, primary care, or urgent care. Strong clinical, communication, organizational, and EHR skills. Ability to work independently and as part of a collaborative care team. Willingness to obtain additional state licensure as needed.
