Skilled Nursing Licensed Vocational Nurse, Full Time

Nursing Care Coordinator

Pay Range: $27.00 - $37.00/hour

Responsible for planning, organizing, implementing, directing, and evaluating direct nursing care provided to residents. Responsible for preparation, delivery and documentation of medications and treatments to assigned residents. Document change of condition, unusual occurrences, weekly summaries, initial assessments, MDS assessments etc. in a thorough and legible manner. Assures notification to physician and responsible party with any change of condition with appropriate documentation. Responsible for knowing and following current federal, state and local standards and assuring compliance with standards functions

Follows facility policies and procedures and federal / state regulation. Participates in facility quality management program. Follows oral and written instructions accurately. Keeps supervisor informed of work priorities and problems. Responds appropriately to emergency situations and disasters. Provides a safe environment for patients, visitors and staff. Assures security of environment to prevent exit of wandering residents. Implement the facility procedure for elopement if any resident leaves the facility without permission. Follows facility standards relating to customer service. Maintains confidentiality of residents and other work-related issues. Assures that all residents are treated with respect and dignity. Follows facility rules of conduct. Attends in-service related to work and completes required in-service annually; attends outside classes to enhance role and professional standards, as appropriate. Provides timely notification of illness/absences to appropriate supervisor following departmental policy and timeline. Attends facility meetings as assigned. Follows dress code for work area. Performs other duties as assigned.

Ability to perform nursing care per current community standard for long term care. Interact effectively with residents, visitors, administration, physicians, volunteers and staff; Knowledge of State and Federal laws and regulations governing Long Term Care Ability to read, write and speak English in a clear manner. Ability to understand and respond appropriately to cultural diversity.

Graduate from an accredited nursing program. Charge nurse experience is preferred. Currently licensed as LVN to practice in the State of California Basic Life Support (BLS) certification
