Key Responsibilities

• Conduct thorough patient evaluations, developing personalized treatment plans based on individual assessments of functional limitations and disabilities.

• Implement evidence-based treatment protocols tailored to each patient's unique presentation, ensuring effective regimens to enhance mobility, strength, and pain relief.

• Document patient care activities in a timely and accurate manner, maintaining comprehensive progress notes, evaluation records, and discharge summaries in compliance with regulatory requirements.

• Collaborate with interdisciplinary healthcare professionals as needed, actively modifying care plans based on patient progress and changing conditions.

• Participate in discharge planning and follow-up care, facilitating smooth transitions to home or other care settings for continued patient recovery.

• Engage in ongoing training to ensure compliance with corporate policies and state laws governing physical therapy practices.

• Attend and contribute to team meetings, conferences, and training sessions, promoting a culture of continuous improvement and shared learning.

Patient Population / Client Focus

The Transformative Therapy Center serves a diverse patient population with various rehabilitation needs, including post-operative recovery, sports injuries, neurological conditions, and chronic pain management. Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• A valid state license to practice as a physical therapist in Virginia.

• Current CPR certification.

Preferred Experience

• A minimum of 2 years of relevant experience in a clinical setting, particularly within the rehabilitation field.
