Responsibilities Deliver exceptional clinical care to patients and support functions related to patient care activities. Actively engage and manage patient panels in partnership with the Nurse Practitioner; act as an extension of the NP to support delivery of high-touch, high-quality care. Advocate for patients and help navigate necessary healthcare resources to support their care plan. Ensure efficient clinic flow by rooming patients in a timely manner and assisting the provider with staying on schedule. Complete pre-visit activities including gathering and verifying health information, preparing charts, prepping exam rooms, and escorting patients from the lounge to the room. Conduct visit activities including taking vitals, performing screenings, recording histories, reviewing medications, and preparing patients for procedures or tests. Provide patient education on preventative measures, lifestyle management, and disease management. Under the direction of the Provider, perform visits such as administering vaccinations and procedures (e.g., ABI, spirometry, EKGs). Oversee the laboratory and related activities including reviewing lab orders, drawing blood, collecting samples, and preparing specimens for shipment or analysis. Support check-out and post-visit activities; facilitate referrals, care coordination, and follow-up outreach to patients and caregivers as appropriate. Assist patients with scheduling follow-up visits, imaging, or labs; handle pre-authorization requests and medical refill processing as required. Maintain efficient patient flow and assist with back-office tasks as needed. Handle back-office phone calls. Quality, Safety, and Compliance Promote clinical and safety standards to drive high-quality care for patients. Maintain equipment, stock and clean exam rooms, and dispose of contaminated items per protocol. Disinfect treatment rooms after examinations; maintain a safe, secure, and healthy work environment by following standards and procedures. Maintain logs and checks (e.g., refrigerator temperatures, emergency medications, expired medications, oxygen) per protocols. Responsible for immunization management, vaccine handling, and adherence to the vaccine management plan. Responsible for medication inventory management and patient care quality KPIs. Collaborate and communicate with all team members to ensure patient needs are met and patient flow is smooth.
