Ambulatory RN Care Coordinator - Lung Care Center

Location: Advanced Lung Care Center – 8925 Colonial Center Drive, Suite 2002, Fort Myers, FL 33905 Department: LPG Advanced Lung Care Center Work Type: Full Time Shift: Shift 1 – 7:00 AM to 5:30 PM Pay Rate: $31.78 – $42.90 per hour Summary The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, promote, and maintain the optimal health status of assigned patients. This position works in collaboration with the patient, physician, and care team in the office setting and serves in an advocacy role to ensure delivery of quality, efficient, and cost-effective patient‑centric health care services. The case load consists of moderate to high risk patients. Requirements Educational Requirements Required: Associate’s Nursing degree Preferred: Bachelor’s Nursing degree Experience Requirements Required: Minimum 3 years of clinical nursing Additional Requirements Minimum 3 years in a clinical setting with emphasis on patient education and chronic disease management; home health, skilled nursing, and medical‑surgical nursing required.
