Position Title: Primary Care Physician (PCP) – Community-Based Behavioral Health
Department: Medical / Behavioral Health Services
Reports To: Chief Executive Officer / Clinical Director
Employment Classification: Contract / 1099 or Other Applicable Classification
Service Area: Fulton, DeKalb, Cobb, Gwinnett Counties and surrounding areas
Position Type: Community-Based / Field-Based
POSITION SUMMARY
Premier Comprehensive Services, LLC (PCS) is a community behavioral health organization providing person-centered, recovery-oriented services to children, adolescents, adults, and families. The Primary Care Physician (PCP) will provide comprehensive primary medical care to individuals receiving services through PCS, with an emphasis on integrating physical healthcare with behavioral health services. The physician will provide care in community-based settings , which may include individual homes, community locations, PCS facilities, and other approved settings. The physician will work collaboratively with PCS behavioral health professionals to identify and address medical conditions that may affect an individual's overall health, functioning, and behavioral health. This position is ideal for a physician who is comfortable providing care outside of a traditional medical office and is committed to serving individuals who may experience barriers to accessing traditional healthcare.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Primary Care Services
- Conduct comprehensive medical evaluations and physical examinations.
- Diagnose, evaluate, and treat acute and chronic medical conditions within the physician's scope of practice.
- Develop individualized treatment and healthcare plans.
- Provide preventive healthcare, health screenings, and patient education.
- Monitor and manage chronic health conditions.
- Order, review, and interpret appropriate laboratory and diagnostic testing.
- Prescribe medications as clinically appropriate and within applicable laws and regulations.
- Provide appropriate follow-up and continuity of care.
- Identify medical concerns requiring referral to specialists or higher levels of care.
- Provide appropriate medical referrals and coordinate follow-up care.
- Recognize and address health conditions that may affect an individual's behavioral health or overall functioning.
- Provide primary care in appropriate community-based settings, including homes, community locations, PCS facilities, and other approved locations.
- Travel throughout the PCS service area as needed to provide accessible healthcare.
- Evaluate individuals in their natural environments when appropriate.
- Help reduce barriers to healthcare access for individuals who may have difficulty accessing traditional medical offices.
- Promote preventive care, healthy behaviors, medication adherence, and overall wellness.
- Educate individuals and caregivers regarding health conditions, treatment recommendations, medications, and preventive healthcare.
- Collaborate with PCS clinicians, nurses, assessors, peer specialists, and other members of the treatment team.
- Recognize the interaction between physical health and behavioral health.
- Communicate relevant medical findings and recommendations to authorized members of the treatment team.
- Participate in treatment planning and case consultation when appropriate.
- Identify medical or medication-related concerns that may contribute to changes in an individual's behavioral or mental health presentation.
- Support coordination between primary care, behavioral health, specialty care, and other healthcare providers.
- Respect the scope of practice and responsibilities of behavioral health professionals and other members of the treatment team.
- Coordinate medical services with external healthcare providers and specialists when appropriate.
- Facilitate appropriate referrals and follow-up.
- Assist individuals in navigating healthcare resources.
- Communicate with authorized family members, caregivers, healthcare providers, and other involved parties in accordance with applicable releases and privacy requirements.
- Promote continuity of care and appropriate communication among providers.
The physician is responsible for completing accurate, timely, and clinically appropriate documentation for all services provided. Responsibilities include:
- Documenting medical evaluations, diagnoses, treatment plans, interventions, medications, referrals, and follow-up recommendations.
- Completing documentation in Notenetic or another PCS-approved electronic medical record.
- Ensuring documentation accurately reflects the services provided and the individual's clinical presentation.
- Maintaining confidentiality and protecting protected health information.
- Completing documentation within PCS, payer, and applicable regulatory timeframes.
- Maintaining appropriate medical records and supporting documentation.
- Never falsifying, duplicating, backdating, or misrepresenting medical services.
The physician will:
- Practice in accordance with applicable federal and Georgia laws, regulations, professional standards, and payer requirements.
- Maintain appropriate medical standards of care.
- Identify and appropriately respond to medical emergencies.
- Follow PCS policies and procedures applicable to medical services.
- Report significant medical, safety, abuse, neglect, or exploitation concerns in accordance with applicable requirements.
- Participate in quality improvement activities as appropriate.
- Maintain professional boundaries and ethical standards.
- Protect patient confidentiality and comply with HIPAA requirements.
- Refer individuals for emergency or higher-level medical care when clinically indicated.
This is a community-based physician position and is not intended to be an office-only role. The physician must be willing and able to travel to assigned locations throughout the PCS service area. Community locations may include:
- Individual residences
- PCS locations
- Community centers
- Healthcare facilities
- Schools or educational settings when appropriate
- Other approved community locations
REQUIRED QUALIFICATIONS
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Active, unrestricted Georgia physician license appropriate for the clinical services provided. Georgia requires a full physician license for clinical practice; an administrative medical license is intended for non-clinical administrative roles.
- Completion of an accredited physician residency program appropriate to the physician's area of primary care.
- Board certification or board eligibility in Family Medicine, Internal Medicine, Pediatrics, or another appropriate primary-care specialty .
- Current National Provider Identifier (NPI) .
- Current DEA registration , if prescribing controlled substances is part of the physician's responsibilities.
- Current BLS/CPR certification .
- Ability to obtain and maintain all required payer and organizational credentialing.
- Ability to meet applicable malpractice/liability insurance requirements.
- Ability to pass applicable background and credentialing requirements.
- Demonstrated ability to provide comprehensive primary care.
- Willingness and ability to provide services in community-based settings.
- Reliable transportation and ability to travel throughout the assigned service area.
PREFERRED QUALIFICATIONS
- Experience providing primary care in a community-based, home-based, mobile, federally qualified health center, underserved, or similar setting.
- Experience working with individuals with behavioral health or co-occurring medical and behavioral health needs.
- Experience working with adults, children, adolescents, and/or families.
- Experience coordinating care with behavioral health professionals.
- Knowledge of community healthcare and social-service resources.
- Experience working with Medicaid and other managed-care populations.
- Experience with electronic medical records.
- Strong communication, clinical judgment, and care-coordination skills.
- Commitment to serving individuals who may experience barriers to traditional healthcare access.
The physician is expected to:
- Provide compassionate, respectful, person-centered medical care.
- Maintain professional and ethical standards.
- Exercise independent clinical judgment within the physician's scope of practice.
- Maintain appropriate professional boundaries.
- Communicate effectively with patients, families, caregivers, and PCS staff.
- Maintain accurate and timely medical documentation.
- Attend required meetings and participate in appropriate clinical consultation.
- Maintain all required licenses, certifications, registrations, and credentials.
- Complete required continuing medical education and professional development.
- Follow PCS policies and applicable federal, state, DBHDD, Medicaid, and payer requirements.
Performance will be evaluated based upon:
- Quality and appropriateness of medical care
- Clinical judgment and patient safety
- Timeliness and accuracy of documentation
- Patient access and continuity of care
- Appropriate management of acute and chronic conditions
- Coordination with behavioral health and medical providers
- Compliance with PCS policies and applicable regulations
- Professionalism and communication
- Completion of required credentialing and training
- Reliability and availability for assigned community services
The physician must be able to function effectively in a variety of community and healthcare environments and may be required to sit, stand, walk, travel between locations, and provide care in homes, community facilities, PCS locations, and other approved settings. The position may require flexibility in scheduling based upon patient needs and community-based service requirements.

This position is specifically designed for a Primary Care Physician who is willing to bring medical care into the community.
PCS is committed to addressing the whole person by connecting primary medical care with behavioral health services. The physician will play an important role in increasing access to healthcare, identifying medical conditions that may affect behavioral health, coordinating care, and helping individuals achieve improved overall health and wellness.
