This role leads an interdisciplinary team and ensures services align with physician orders and individualized care plans.Key ResponsibilitiesManage clinical staff and daily operations in a home health settingDevelop and oversee plans of care (POC) and patient outcomesReview referrals and assign clinicians based on patient needsEnsure compliance with CMS, Medicare, and agency standardsConduct chart audits, case reviews, and case conferencesCollaborate with physicians and support care coordinationParticipate in QAPI and performance improvement programsAssist with hiring, training, and staff developmentQualificationsLicensed RN, PT, OT, SLP, or MSW (active state license)2+ years home health experience requiredSupervisory or leadership experience preferredStrong knowledge of home health regulations & documentationWhat makes us unique?
