Primary Care Physician, Partner (PCPP) In this role the PCPP: Demonstrates accountability for outcomes, delivering strong clinical care and cost‑effectiveness for a risk‑adjusted panel of at least 400 patients. Acts as a health coach, helping patients set short‑ and long‑term health goals, partnering with them to achieve those goals and frequently following up. Simplifies and prioritizes care so that behavior change is actionable for patients and for the care team. Responsibilities Independently provide care for patients with acute and chronic illnesses in older adults. Take full accountability for patient care and outcomes; seek specialist consultation when needed while remaining responsible for detailed care. Engage with the hospitalist whenever a patient is hospitalized, regardless of hospitalist affiliation. Perform assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. Lead the care team consisting of a medical assistant, care facilitator, and care coordinator for patients able to come to the office. For patients unable to come to the office (in‑hospital, SNF, LTC, or homebound), collaborate with the transitional care team, case managers, acute and transitional‑care physicians, and other resources as available. Play an active role in center management, covering for other providers when needed and helping recruit patients and additional providers to the market. Supervise, collaborate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions while employed with the company. Perform other duties as assigned and modified at the manager’s discretion. Knowledge, Skills, and Abilities Availability and Accessibility: be available for walk‑in visits, phone calls, and messages in a timely manner. Service Orientation: provide care they would want for a family member or for themselves. Evidence‑Based Medicine: stay updated on evidence‑based medicine and lifestyle/lifestyle interventions that impact outcomes. Physician Leadership: continuously develop leadership skills for the benefit of patients, the team, the center, and the company. Quality: work with the care team to achieve high‑quality outcomes and quality measures, and continually improve practice and operations. Influence: competently and compassionately influence patients, teams, and self to achieve the best outcomes. Self‑Care: care for oneself physically, mentally, socially, and spiritually to sustain wellness. Proficiency in Microsoft Office Suite and other word‑processing, spreadsheet, database, e‑mail, and presentation software used by the company. Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training. Fluency in English, verbal and written; additional language fluency may be required in some centers. Use independent judgment in daily practice. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current, active MD licensure in the state of employment. A minimum of 6 years of clinical experience in geriatric, adult, or family practice, including 2 years of ChenMed PCP, Senior Partner experience. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty preferred; Board eligibility required. Maintain board certification through MOC, CME, and/or retakes as required. Current DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.
