Title: Physical Therapist (PT)
Employment Type: Full-Time / Permanent
Location: Greenbrae, CA 94904

Schedule: 8:30 AM – 4:30 PM

Pay: Hourly Rate: $63-$66/hour (Based on experience) + Benefits

• $10,000 Sign-On Bonus

Facility Setting:

• Skilled Nursing Facility (SNF)
• Assisted Living (AL)
• Independent Living (IL)

-> Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled PT interventions to improve mobility, strength, balance, safety, and functional independence.
• Treat patients across SNF, assisted living, and independent living environments.
• Manage a diverse caseload including orthopedic, neurological, post-acute, and medically complex conditions.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary teams.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on therapy goals, safety techniques, and home exercise programs.
• Participate in care planning and maintain compliance with Medicare and regulatory requirements.

-> Qualifications:

• Active California Physical Therapist (PT) license .
• Graduate from an accredited Physical Therapy program (DPT preferred).
• Experience in SNF, Long-Term Care, Assisted Living, Independent Living, or Post-Acute Rehabilitation preferred.
• Strong clinical judgment, communication, and documentation skills.
• CPR/BLS certification preferred.
