Start Of Care/Admission Registered Nurse (RN) For Home HealthPrimarily responsible to plan, organize, direct home care services. Experience in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Wage range: $88,000-$90,000 annually, depending on experienceService Area: Twin Cities Metro Area. Provides a complete physical assessment and history of current and previous illness(es).Initiates the plan of careUses health assessment data to determine nursing diagnosis.Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.Counsels the patient and family in meeting nursing and related needs.Provides health care instructions to the patient as appropriate per assessment and plan of care.Identifies discharge planning needs as part of the care plan development.Completes necessary paperwork accurate at admission and submits to medical records the same day.Completes emergency preparedness plan with client/caregiver at admission visitCommunicates with IDT post admission or non-admission including client status and a summary of the plan of care for client.Ensure positive/professional communication on teamIntroduction/Establish RapportCustomer service resolution; escalate concernsPrepares clinical notes and updates the primary physicianCommunicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's ordersCommunicates with community health related persons to coordinate the care planHonors Client Confidentiality, Rights, Privacy, and Reporting Maltreatment ExpectationsUnderstands and Accepts Clients' DiagnosisUnderstands and accepts emotional needs of clientAccepts unique client symptoms and behaviorsFollows all emergency preparedness and response proceduresIdentifies home safety issues and cascades info to the IDTNotifies IDT of emergency preparedness planDemonstrates proper procedures for handling, lifting, and transfersOverview of Process, Types of Reports and FormsEnters complaints, occurrences into EMR as necessary at skilled visitsEnsures education on how to use equipment safely is completed at admission and that medical supplies are ordered.Plots necessary supervisory visits per regulationsThoroughly reconciles all medicationsCompletes all documentation by 10am the day following the admission or skilled visitComplete orientation and annual training requirements.Promote safety in the workplace through real time coaching, participation in safety awareness trainingParticipate in After Hours on-call programOther duties as assignedQualifications:2 years' RN experience in health care (preferably home care or hospice)Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferredExperience in customer service/direct member contact field preferredExperience supervising caregiversKnowledge of Medicare Home Health & Hospice regulationsKnowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.Graduate of an accredited school of nursingCurrent Minnesota Nursing LicenseMust pass a MVR check based on Lifespark company policyContact Lifespark Now!
