Transform Youth and Family Counseling - Southern Oregon Psychiatric Mental Health Nurse Practitioner (PMHNP) Compensation: From $110/hour (Fee compensation split) 1099 Independent Contractor Position Employment Type: Part-Time to start Location: Grants Pass or Medford, Oregon (In person or hybrid) Transform Youth and Family Counseling is a modern, inclusive, youth-centered outpatient behavioral health practice serving children, teens, and young adults (ages 6-24). We are expanding our services and seeking a Licensed Psychiatric Nurse Practitioner to join our collaborative, values-driven clinical team. *Position opening at either our Grants Pass OR Medford office. What We Offer Compensation fee split (1099 position) Flexible scheduling (start with a part-time caseload with option to increase to full-time) Beautiful, modern office spaces in Grants Pass and Medford Dedicated administrative support (scheduling, phone triage) EHR + AI charting assistance Oregon student loan forgiveness approved site Supportive, trauma-informed, collaborative team environment Conduct psychiatric diagnostic evaluations for youth ages 6-24 and for adults of youth that are receiving services with the practice Provide ongoing medication management follow-ups Collaborate closely with therapists at our practice and participate in team-based health care services Participate in team meetings and case consultation Maintain accurate, timely documentation in the EHR Provide culturally responsive, trauma-informed care Licensed Nurse Practitioner, Certified in Psychiatric & Mental Health (PMHNP Required) in the state of Oregon Active Oregon DEA number (or ability to obtain prior to seeing patients) Experience working with youth OR strong interest in developing youth expertise Ability to work effectively within a collaborative care model About Transform Youth & Family Counseling At Transform, our mission is to provide a safe, inclusive, and affirming space for youth and families to access high-quality mental health services.
