Are you a compassionate Social Worker with a heart for service and a calling to support patients and families during life’s most tender moments?

We are seeking a dedicated Hospice Social Worker to become an essential part of our caring and supportive interdisciplinary team. You’ll provide emotional support, guidance, crisis intervention, and resource coordination to patients and their families as they navigate end-of-life care.

• Conduct psychosocial assessments and provide ongoing emotional support

• Develop individualized care plans alongside the interdisciplinary team

• Help patients and families access community resources and supportive services

• Assist with advance directives and end-of-life planning

• Provide crisis intervention and compassionate guidance

• Offer bereavement support to families

• Maintain accurate, timely, and professional documentation

• Advocate for the dignity, comfort, and wishes of every patient

• Bachelor’s or Master’s degree in Social Work ( BSW/MSW )

• Valid Pennsylvania Social Work license

• Pennsylvania residency

• Hospice or healthcare experience preferred

• Strong communication and interpersonal skills

• Empathy, resilience, and professionalism
