Job Summary The Western New York VA Medical Center is seeking a Registered Nurse to serve as a Community Care Coordinator on the Care in the Community Team, located at the historic Buffalo VA Medical Center. The role involves a streamlined approach to receiving, triaging, directing timely coordinated care, and assessing, planning, implementing, and evaluating the effectiveness of veterans' care. Responsibilities Receive, triage, and direct timely coordinated care for veterans. Assess, plan, implement, and evaluate the effectiveness of veteran care. Basic Requirements English Language Proficiency in basic written and spoken English. Graduate of an accredited professional nursing school (ACEN or CCNE) or equivalent program. Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, or the District of Columbia. Preferred Experience BSN degree. Experience in community care and/or consult coordination. Experience in care management. Physical Requirements Visual acuity, keen hearing, clear speech, and manual dexterity. Ability to walk, stand, stoop, sit, bend, pull, push, and transfer patients; mechanical lifts are provided. Capability to don protective clothing during isolation or invasive procedures; annual health requirements (e.g., TB screening) must be completed. #J-18808-Ljbffr
