Job Summary:
Seeking a dedicated Physical Therapist to provide quality rehabilitation services in a skilled nursing facility setting. The ideal candidate will assess patient needs and develop appropriate treatment plans to promote recovery and independence.

Responsibilities:

• Evaluate patients' physical conditions and establish treatment goals.
• Develop and implement individualized therapy plans based on patient assessments.
• Monitor patient progress and adjust therapy techniques accordingly.
• Educate patients and families on therapeutic exercises and injury prevention.
• Document treatment sessions and maintain accurate patient records.
• Collaborate with the healthcare team to coordinate care and optimize outcomes.

Qualifications:

• Hold a valid state license or certification as a Physical Therapist.
• Demonstrate previous experience working in a skilled nursing facility or similar setting.
• Possess strong assessment and clinical decision-making skills.
• Exhibit excellent communication and interpersonal abilities.
• Be able to work independently and as part of a multidisciplinary team.
