Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 2 years in acute care case management/discharge planning License: CA state license required; pending license accepted (must be licensed upon start date) Certifications: Required competencies in medical necessity determination, care coordination, and discharge planning Must-Have: - Ability to organize and participate in family meetings - Experience with assessing patients and developing treatment plans - Competent in arranging DME, SNF, and ambulance transport - Knowledge of adoption/foster care, drug/alcohol abuse treatment options, and mental health crisis management - Familiarity with guidelines of Medicare and Medi-Cal - Must be available for weekend shifts and rotating holiday coverage Description: We are seeking a dedicated and skilled Registered Nurse specializing in case management. This position involves working in an acute care facility with a focus on care coordination and discharge planning. The role requires managing a patient load of up to 25 in various stages of care. The ideal candidate should have a strong understanding of medical necessity criteria and experience with care coordination. The case manager will participate in family meetings to discuss treatment plans and the needs of patients. The candidate must possess excellent organizational skills and be able to work independently. Knowledge of community resources and eligibility for services is essential.
