Key Responsibilities

• Collaborate with physicians, nursing, social workers, and support staff to facilitate efficient and safe patient throughput
• Conduct comprehensive assessments to identify clinical, psychosocial, and post-acute care needs
• Develop individualized discharge plans, including coordination of skilled nursing, rehabilitation, home health, hospice, and durable medical equipment (DME)
• Serve as a liaison between patients, families, and community resources, ensuring continuity of care
• Advocate for patient rights while balancing utilization review and cost-effective care practices
• Monitor length of stay (LOS), readmission risk, and quality metrics
• Maintain documentation in compliance with regulatory standards and hospital policy
• Communicate clearly with payers to resolve insurance issues and support appeals
• Assist in regulatory tasks such as issuing Medicare Important Messages and Non-Coverage Notices

Requirements

Education:

• RN: Graduate of an accredited nursing program (BSN preferred)
• OR Social Work: Masters in social work (MSW) from an accredited institution

Licensure & Certifications:

• RN: Active RN license in New Mexico
• Social Work: LMSW, LISW, or LCSW in New Mexico
• Case Management certification (ACM, CCM, or RN-BC) preferred
• BLS certification required

Experience:

• Minimum of 2 years in healthcare, with 1+ year in an acute care setting preferred
• Familiarity with discharge planning, medical necessity criteria, and community resources

Skills:

• Strong communication, critical thinking, and time-management skills
• Ability to multitask and adapt in a fast-paced, patient-first environment
• Proficient in EMR and Microsoft Office platforms
