In this role, you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions, support transitional care planning, and contribute to quality-improvement initiatives within your designed facilities.Core Responsibilities:Collaborate with Trumen physicians and facility medical directors to coordinate patient careConduct comprehensive clinical assessments and manage treatment plansSupport transitional care planning to help prevent avoidable hospital readmissionsReconcile medications and provide patient education consistent with scope of practiceFoster positive working relationships with facility leadership, clinical staff, and care coordinatorsCollaborate as applicable Trumen-affiliated value-based and quality improvement initiativesServe as a liaison between Trumen and designated skilled nursing facilities (SNFs)Maintain accurate and timely documentation within the EMRMaintain an on-site presence consistent with your agreed schedule and facility needsQualifications:Active Nurse Practitioner license in the state of TexasMinimum of 1 year experience as a Nurse Practitioner, preferably in a post-acute, SNF, or geriatric care settingAbility to work independently while effectively collaborating with the care teamStrong interpersonal skills and a passion for caring for complex patientsReliable transportation within the Kerrville areaAbout Trumen Physicians + Associates:Founded in 2010, Trumen is a physician-led, provider-centric organization focused on improving outcomes for patients in post-acute and transitional care settings.
