Certified Community Health Worker (CCHW)

Remote (Florida Residents Only) | $23/hour


We are seeking a compassionate and motivated Certified Community Health Worker (CCHW) to join a growing healthcare team serving communities across Florida.


Requirements:

• Active Florida Certified Community Health Worker (CCHW) certification or currently enrolled in a certification program.
• Candidates with an active Community Health Worker certification from another state are also encouraged to apply.
• Must reside in Florida.
• Strong communication, organization, and problem-solving skills.
• Ability to build rapport with diverse populations while providing compassionate support.

Responsibilities:

• Connect members with community resources and healthcare services.
• Conduct outreach and provide education on available programs.
• Maintain accurate documentation and case notes.
• Collaborate with care teams to support member needs and improve health outcomes.
• Assist members in overcoming barriers to accessing care and social services.

This is a fully remote opportunity offering $23/hour for candidates passionate about making a difference in their communities.
