Certified Nurse Practitioner – Family Health Center (Clinic Setting)About UsVNA Community Services has provided compassionate, community-centered care throughout Montgomery, Bucks, and Philadelphia Counties for over a century.Our mission is to empower individuals and families to live with dignity, independence, and confidence - delivering high-quality primary care and supportive services across all ages.As we continue to grow, we are seeking a dedicated, experienced Certified Nurse Practitioner to join our Family Health Center team and help expand access to comprehensive, patient-centered care.Position: Certified Nurse Practitioner (Full-time or Part-time)Location: [Insert Clinic Location - e.g., Abington, Norristown, Pottstown or other VNACS Family Health Center site]Reports to: Clinic Medical Director / Family Health Center LeadershipWhat You'll DoProvide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management.Conduct patient assessments, diagnose illnesses, order/inter­pret diagnostic tests, and develop and implement treatment plans.Prescribe medications and therapies, including managing ongoing medication regimens.Provide patient education, counseling, preventive health guidance, and support for health maintenance.Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care.Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems.Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care.Who You Are - Required QualificationsMaster 's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program.Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult- gerontology).Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations.Excellent communication skills, with a patient-centered and compassionate approach.Capacity to work independently and collaboratively as part of a multidisciplinary care team.Familiarity with electronic health records (EHR) and documentation best practices.What We Offer - Why Join UsOpportunity to make a meaningful difference in the lives of individuals and families across our community, including diverse populations and underserved areas.Being part of a non-profit organization with over 100 years of history and a strong commitment to community health.A supportive, collaborative clinic environment that values compassion, respect, and professional integrity.Competitive compensation and benefits (to be defined based on full-time/part-time and experience level).Opportunities for professional growth, continuing education, and career development through our integrated care model and diverse service offerings (primary care, home care coordination, chronic disease management, outreach). You may also contact us at View phone number on ziprecruiter.com for more information.VNA Community Services is an equal opportunity employer.
