Care Coordinator – Children's Long-Term Support (CLTS) Program Waukesha County, WI Full-Time | M–F, First Shift | Remote Flexibility $23.70/hr Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County ! The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services. Please note: This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area.
