Physical Therapist Assistant

Overview Presbyterian Homes & Services (PHS) is a nonprofit, faith‑based organization that provides high‑quality housing, care, and services options for older adults. The Home & Community based division of PHS, Optage, is hiring a Full‑Time Physical Therapist Assistant to serve in home‑health care for communities in the south Twin Cities metro area. Schedule / Hours Monday through Friday, full‑time (up to 30 hours per week) with a $1,000 hiring bonus. Responsibilities Provide treatment per the established plan of care and in accordance with physician orders. Assist in identifying goals, prioritizing treatment, and selecting appropriate interventions. Continuously assess changes in functional deficits and adjust treatment as appropriate. Provide education to residents, clients, families, caregivers, and staff. Collaborate with therapists on observations, progress toward goals, and recommended updates to plan of care and discharge. Provide accurate and timely documentation & billing in electronic medical records. Demonstrate understanding and knowledge of payer sources and regulatory requirements. Coordinate with therapists on supervisory visits per regulations. Communicate effectively with residents, clients, families, caregivers, physicians, referral sources, and the interdisciplinary team. Qualifications Graduate from an accredited Physical Therapist Assistant program. Current licensure in the state(s) of practice as required. Demonstrated compatibility with PHS’s mission and operating philosophies. Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers. Pay Range Estimated starting pay range: $30.24 - $34.57 per hour. Actual pay depends on qualifications and market conditions.
