Patient Care Coordinator

To support patients in navigating their healthcare journey, the full-time remote Patient Care Coordinator will manage referrals, medications, and treatment plans while ensuring timely and compassionate service. Key responsibilities: Serve as the primary point of contact for patient inquiries regarding referrals, imaging, lab work, and medications Collaborate with healthcare providers and facilities to ensure effective coordination of patient care Manage scheduling requests and maintain accurate patient records and documentation Required qualifications: High school diploma or GED required; Bachelor's degree preferred 2+ years of customer service or healthcare experience preferred 1+ years of experience with EHR systems, preferably athenaOne Strong understanding of medical terminology and insurance processes required Experience in a remote work environment, especially with high-volume calls, preferred
