Summary of Duties:

The Permanent Supportive Housing (PSH) Case Manager provides housing-focused, client-centered case management services to individuals and families residing in permanent supportive housing who have experienced chronic homelessness and may have behavioral health, physical health, substance use, or other complex service needs. The PSH Case Manager utilizes evidence-based and trauma-informed approaches to support housing stability, improve quality of life, increase self-sufficiency, and promote community integration.

Services include comprehensive assessments, individualized service planning, crisis intervention, benefits navigation, linkage to healthcare and behavioral health services, and ongoing support tailored to each participant's goals. The PSH Case Manager applies best practices such as Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care, while collaborating with community partners and interdisciplinary teams to ensure residents maintain stable housing and achieve positive outcomes.

Essential Duties and Responsibilities

• Conduct comprehensive intake, assessment, and reassessment of participant strengths, needs, barriers, and progress toward goals.
• Develop, implement, and regularly update individualized service plans in collaboration with participants.
• Provide ongoing case management services focused on housing retention, stability, wellness, and self-sufficiency.
• Conduct regular outreach and engagement with residents through home visits, office visits, phone contacts, and community-based interactions.
• Provide crisis intervention, de-escalation, and safety planning as needed.
• Assist participants in accessing and maintaining public benefits, healthcare, behavioral health services, substance use treatment, employment services, legal resources, and other community supports.
• Facilitate life skills workshops, support groups, and community-building activities that promote resident engagement and independence.
• Advocate on behalf of participants with landlords, healthcare providers, social service agencies, and other community partners.
• Collaborate with property management and multidisciplinary team members to support housing stability while maintaining appropriate confidentiality.
• Maintain accurate, timely, and complete participant records, case notes, service plans, and other required documentation in compliance with agency policies, funder requirements, and best practices.
• Enter and maintain all required client data and service information in the Homeless Management Information System (HMIS) and other agency databases.
• Review assigned caseloads regularly to ensure service quality, compliance, and achievement of program outcomes.
• Participate actively in case conferences, interdisciplinary team meetings, staff meetings, trainings, and professional development opportunities.
• Monitor participant progress and evaluate outcomes related to housing stability, health, income, and community integration.
• Maintain knowledge of local resources, housing programs, and community services available to participants.
• Ensure compliance with all applicable federal, state, local, and agency regulations, policies, and procedures.
• Perform other duties as assigned by the supervisor.

Qualifications:

• Basic understanding of the military and the Veterans Administration
• Minimum of 1 year experience working with homeless population or AA degree in Human Services
• Work or related field experience preferred.
• Knowledge of HMIS database a plus!
• Knowledge of Housing First approach a plus!
• Ability to prioritize competing responsibilities and thrive in a challenging situation
• Able to change priorities at a moment’s notice
• Must be able to fluently read, write and communicate in English
  • Must be able to comfortably, both mentally and physically, perform all aspects of job description and other duties as assigned by supervisor

• Must be able to sit comfortably for long periods of time
• Valid California drivers license and up to date vehicle insurance
• Veteran Status a Plus!
