OBJECTIVES: Reporting to the CIS and FFS Program Supervisor, the CIS Transfer Client Clinician is responsible for delivering high-quality, culturally responsive care and case management services to children and families that transfer away from the CIS Program and are awaiting connection to a therapist at the Main Clinic location. This individual will be collaborating with CIS Clinicians to ensure a warm hand off for the client, the intake department ensure seamless connection to treating therapist once available, the medical department to provide crisis intervention and evidence-based consultation and Main Clinic Clinicians upon assignment for best transfer of care. The CIS Transfer Support Clinician will work to meet productivity standards while engaging in ongoing supervision and professional development in accordance with ethical and regulatory guidelines, as appropriate for part time staff members.
DUTIES AND RESPONSIBILITIES :
- Provide high-quality psychotherapy services to children and families from diverse educational, cultural, and socio-economic backgrounds, with a primary responsibility for CIS-Main Clinic transfer and continuity of care,
- Collaborate closely with school administrators, teachers, and all Northside staff, in conjunction with supervisor, to ensure coordinated and effective delivery of the following:
- Support and assist with crisis intervention and risk assessment for students, including participation in safety planning and ongoing monitoring of high-risk cases.
- Coordinate care with psychiatrists and psychiatric nurse practitioners to ensure comprehensive, integrated treatment for assigned clients.
- Coordinate with Intake Department to ensure seamless connection to treating therapist at main clinic location once available as well as managing a transfer tracking system of all clients supported.
- Provide case management services as needed, including collaboration with families, external providers, and community-based support agencies.
- Maintain accurate, timely, and compliant clinical documentation in accordance with clinic standards and regulatory requirements.
- Meet established productivity expectations and maintain a work schedule aligned with client service needs.
- Participate in supervision, team meetings, case conferences, training, and ongoing professional development, while upholding ethical standards, including strict adherence to HIPAA regulations, as appropriate.
- As capacity allows, provide short-term coverage for CIS school sites during staff leave or temporary vacancies.
SPECIFIC SKILLS :
- Demonstrated experience working in mental health settings with children, families, and adults.
- Strong administrative and organizational skills with the ability to effectively coordinate operations within a school-based clinic environment.
- Training and experience in evidence-based treatment models.
- Ability to exercise sound, independent clinical judgment in a fast-paced setting.
- Excellent written and verbal communication skills.
- Strong interpersonal skills to effectively engage with clients, colleagues, supervisors, and school partners.
- Demonstrated cultural competence and ability to work effectively with diverse populations.
- Proficiency in relevant technology and applications, including Microsoft Office 365 (Word, Excel, PowerPoint, Forms, Teams), Zoom, Doxy.me, and electronic health record systems.
- Bilingual in Spanish and English prefer to meet the linguistic and cultural needs of the populations served.

QUALIFICATIONS :
- Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Mental Health Counselor (LMHC).
- Demonstrated experience providing services in a school-based setting preferred.
- Active and unrestricted New York State license to practice.
- Doctoral degree (Psy.D. or Ph.D.) in Psychology from an accredited institution.
- Active and unrestricted New York State license in Psychology.
COMPENSATION AND BENEFITS:
Part time, $50,000 annual salary
