Primary Care Nurse Practitioner / Physician Assistant Adult & Geriatric Care | Sheboygan

Oak Medical SC is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to provide acute and chronic care management for adult and geriatric patients throughout our Sheboygan service area.

As a primary care provider, you'll collaborate with physicians, specialists, and facility care teams to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.

What You'll Do

• Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.
• Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice.
• Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams.
• Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services.
• Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.
• Maintain timely, accurate, and compliant documentation.
• Build strong relationships with patients, families, and facility staff to support exceptional care.
• Perform other duties as assigned.

Qualifications

Required

• Current Nurse Practitioner or Physician Assistant license.
• Active DEA registration.
• BLS certification.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Commitment to compassionate, patient-centered care.

Preferred

• Leadership experience or demonstrated leadership skills.
• Strong organizational and time-management abilities.
• Positive, solution-oriented attitude.
• Experience working with adult and geriatric populations.

Work Setting

Onsite patient care within assisted living, skilled nursing facilities, specialty hospitals, and senior living communities
