Highlights
\n Community Physician Network is seeking an Internal Medicine physician or Endocrinologist with special interest in Type-II Diabetes management to join our new Diabetes Care Clinic in Kokomo, Indiana. The physician in this role will join a multi-disciplinary team (NP, pharmacy, social work, Diabetes educator) in an outpatient setting. Ideal candidates will be collaborative and have a passion for educating patients to improve outcomes and quality of life.

Outpatient-only position

\n

Monday-Friday day schedule

\n

EPIC EMR

\n

Excellent compensation and benefits (options for signing bonus, loan repayment, relocation)

Support of a multi-disciplinary team

Licensure & Certifications

\n

BE/BC in Internal Medicine or Endocrinology

\n

Additional experience/certification in Diabetes care

\n

Licensed or eligible for medical licensure in Indiana
